This is indeed an attempt to gather identifying information.  If you have fallen victim to this scam, please report it at ic3.gov, which is the FBI and its partner's website for reporting internet crime.  Always remember, if something is unsolicited, or if someone offers you something for nothing or very little, it should send off little warning bells.  Also, there is very little jury appeal if and when these people get caught, as the victim often appears greedy, opportunistic, and, more importantly, incredibly stupid.

We just received one of these. It seemed a bit fishy. If they would like this kind of information from someone they would send it directly to that person with a contact number.

We got one too, although the form wasn't attached.  I would think a real IRS letter woudl be addressed to our company rather than just "Dear Sir".
