I recently visited UCLA hospital and they called up me for a health survey.

Whoah -- weird -- exactly one year later I'm reporting again on this number. This time a live (as opposed to recorded, not dead!) woman's voice said she was calling for <my name> regarding a brief health care survey. I am pretty sure I know what doctor's office it is for.

This number called me at 2:43 pm on August 23. Caller ID said "Qualit Data". I was out of town for several days so I am not sure if they left a message or not (my machine is old and does not indicate which message came from which number)

Called today... again from Quality Data Management this time its an offer to lower interest rate for mortgages and to how to take advantage of the government plan. Very annoying this phone calls will place them on my block list.

It's a company called Quality Data Management that conducts surveys for health care providers. They won't leave a message and you can't call them back either, it's just a recording saying that they will try to reach you at another time.
