I received a call from this number, but they stated they were from US Health Group out of texas.  They also said they were part of AWA -Affiliated Workers Assn.  However, the number he used was from Florida and online it seems to belong to a medical group called Leve One- Spoke with a Mike Sutton-877-218-4619  ext:214.  They gave decent rates and made all sorts of claims for the cost of $239.00 monthly.
My problem is when i called back to get more info the caller aswered saying Level One Care or insurance.  This really caught my attention because this was a different name than Mike agve me.  Has anyone heard of this other insurance company-Rates are decent, but want a lot more info before I sign up-Thanks
