Abrazo or Vanguard health, satisfaction survey.

Automated patient survey for Maryvale Hospital (Abraza Health Network), as to how you're feeling after returning home, how long you waited to see a doctor, etc.  Phoenix Baptist Hospital is owned by the same network, so the "800" number for call-back is the same as "Joe Shmoe" listed. Voice main indicated they would call again later, so I called back to the same number on my caller ID and answered a couple of the questions before terminating the call.  Hopefully that will prevent additional call attempts.
