Got the same thing today on our business fax. I am an insurance agent so it's sort of funny.
I had NO idea who the hell 'T' was! Glad I looked up the phone number in Google.

I wouldn't call that number for a free report. Instead review your policy, talk to your agent.

I have been receiving this EXACT FAX since 2007 at my business in Massachusetts, I started working 2 jobs & receive the same EXACT fax over there too. My father even receives it on his number.Every 3 months or so I get the same number, picture, handwritten note, biker on the side etc.etc. NEVER has there been a person on the other line, even in 2007.Now there's a recording stating the number is no longer in service.All of the phones that receive this fax are on the Do Not Call list also. How can we report if there's no received from number, anyone can type a number on a fax.
This should be reported but how?

Received same notice.  Get this type of fax daily.  Call the number and give fake information whenever you can.  It costs THEM for you to call.  Let's see if they like paying for a phony call!

someone faxed my machine with a fake newspaper ad about rising insurance rates, and fake note saying "thought you would find this interesting, F."

At the end, 1-800-380-3602 was given as a number to call to find out more, No Charge!

I will set up a program to call them automatically every five minutes.  When they contact me about it, I will discontinue the calls until they remove my fax machine number from the list.  What they are doiing is harassing fax owners and we should just harass them back.

we are recieving the same faxed document referenced above about once every two days. There is no sender info just the date, time and our fax# at the top of the page. it says to call 1-800-204-5610 24 hours a day for a free recorded message.

Same as all the above.  1-800-380-3602.  If you do a "Reverse Lookup" you get "Qwest" website, which has
nothing to do with this slug.  Qwest is a High-Speed Internet Services Company with the same number, for
some reason.  But the junk is still the same.  :Watch Out!".  "Thought you'd find this interesting" (as though
it came from a friend!).  There is a partial picture of a bicycle rider on the right side.  The whole thing is lopsided and looks as though it came from a newspaper or magazine.  The reason I did not dial the number is that I figured is that the thing is a scam, first, and second if I dial I may get onto some kind of master list whereby I start getting more FAXES.  Responding to the number only lets the sender know that you exist.  He (she) sends out who-knows-how-many of these stupid things randomly, waiting for a bite.  The phone numbers are totally unknown to him (her) and only by a reply can that person further the scam.  You can try to report the number, but be aware that the number may also be the number of a legitimate business, such as QWEST.  To see what I mean, visit the web site:  http://www.qwest.com for particulars.  To get THEIR phone number I did a reverse lookup at http://www.customtollfree.com/toll-free-reverse-lookup/8003803602 and in prominent numbers at the top of the page I noticed the same number as the scam number.  So, if you are going to try to report a number, be careful.  You may be reporting a legitimate number as well.  I can't say how two assignments can occur, but there you have it.

We received a fax with Watch out! Behind Your Back They'll Rob you Blind.  I have called the 877-603-4782 number at the bottom to remove our fax number -- 3 times!! and they keep sending it.  I tried to fax it back to them at the number on the top of the fax 800-219 0683 and it is a directory assitance number.

Our business office received the same unwanted insurance "article" with a handwritten note as thought it was from someone who was known to the office stating "thought you'd find this interesting".  I went on the FCC's website and reported it!  We are a very busy medical office and have tons of faxes coming through daily with important patient information.  These inane and unsolicited "faxers" need to stop!  We are not going to take it lightly in our office any longer and are reporting those who do so!

Also: The banner at the top of the fax had no ID (company name or senders fax number) which I believe is technically a violation of RI state law(if not Federal as well). RI also has laws against unsolicited faxes.
