A person claiming to be Matt "Pull" left a message on my answering machine stating that it was not a solicitation, but that he was looking for my parents.  His return number was 1-800-905-1134.  I get calls from Formax as well, but they come in spurts (i.e., they'll call several times a day for days in a row and then stop for a while).  In between the Formax calls I get several "Out of Area" calls.

someone from global international called me about information about government grants and wanted my credit card information for 4.99 shipping and handling. then when I said I wasn't interested they hung up but called right back and so I listened then I got suspicious because they were so insistent about a 39.95 charge for a coach to help fill out the forms after a seven day free trial when I said I wanted to cancel the whole transaction and told them I was notifying my bank they hung up.Please help should I cancel my debit card since they have the number.

I was having the same problem with Formax calling me about Quest Diagnostics....DO NOT GIVE FORMAX ANY MONEY!!!!  If you have outstanding bills contact Quest Diagnostics directly.  I have spoken to the QD's  corporate office and made them aware of how inappropriate and unprofessional the Formax representatives are.  There are federal laws protecting us from harassment.  There are rules and regulations that collection agencies must abide by.  I contacted the federal trade commission and made a complaint (877-382-4357).  Formax uses threatening  tactics and verbal harassment to scare people into paying their bills.  This is against the law.

I posted the comment above this one.  The same number, 215-946-0749, called me again and I wasn't paying attention so I picked it up this time.  I can't remember the name the guy gave, but he said he was calling from Formax and was trying to collect on a bill from Quest Diagnostics (which is really a company I get bills from) from 2005.  He claimed he had sent me several bills to an outdated address (which he told me, and it really was an outdated address for me), so I thought it might be legitimate.  But he was very pushy and wouldn't give me any other options to pay other than by phone (either checking account or credit card), saying that it was urgent and this would hurt my credit report.  I finally realized it was a fraud when he offered to cut the bill in half if I would just pay something.  Right - like real insurance agencies would do that.  I then hung up, and have put the number in my phone as "DO NOT PICK UP."

I got a call from 215-946-0749.  I didn't recognize the number, so I let it go to voice mail.  Joe Stevens left a voicemail specifically using my name, saying please call him back at 1-800-905-1134, but not stating who he was or why he was calling.  After reading the above posts, I'm not going to call him back.  It seems very sketchy.

I received a call from 215-946-0749 directing me to call 1-800-905-1134 in regards to a past due payment.  After talking with my providers no information was ever sent to a collection agency.  The man I spoke with from the 1800 # (supposedly Formax Financial) said his name was Joe Stevens and was very rude and demanding to get any payment information over the phone.  He even said he would "cut the bill in half" to help me out as long as I paid it this instant.  Very sketchy especially since my providers have no record of any information ever being sent to collection.  Beware of the Formax representative that uses these pushy tactics.

We have had this number on our caller ID several times. One day when we did answer the phone, they said they are a collection agency and was looking to take a payment over the phone. If not they will take action against us in the court.
