JUST CHECKED MY BANK ACCOUNT ONLINE AND NOTICE A CHARGE OF 9.85 ON MY CARD FROM FORM LIMITED IN FLORIDA. I'M IN THE PROCESS OF DISPUTING IT WITH MY BANK. I CALLED THE NUMBR AND ITS JUST AN AUTOMATED SYSTEM THAT FORWARDS ME TO A VOICEMAIL.

I had the same problem: a charge for $9.75 and I had no idea what it was. I called the Credit Card company and they sorted it out.  Sure enough, they said it was something like a DVD rental.  I'm keeping my eye on my statements just in case it happens again.

Hi!  I just noticed a 9.80 charge on a credit card I use all the time. I got the statement in the mail today and called my credit card immediately.  I ran an internet search for Form Limited, but didn't get anywhere.  I also called the number listed on the statement but got the message that I have read in the previous postings.  
When I did an internet search for the telephone number, I found the postings on this site.
This is scary.  I have never heard of this co., nor ordered any dvd's online.  My card has been cancelled due to this transaction.  I hope this gets some public attention as it seems to be spreading.

I never received a call to my knowledge, just the charge showing up on my credit card.

I didn't get a call, but my checking account was charged $9.75 by Form Limited, Coral Gables, FL.  I called my bank and they cancelled my card and reissued it with a new number.

I haven't gotten any calls yet, but I did call the number listed above. I got a $9.75 charge on my personal debit card today. The bank is disputing it right now and is issuing a new card.
I had just used Paypal and then this other charge showed up. I have also called Paypal and let them know about Form Limited (bastards!)and gave them this website address to reference. I hope they catch the slime balls!!

Found the same $9.85 debit and this phone number, Washington Mutual, Seattle area, on my May bank statemnet.  I've never heard of them, and have never done business with them.  The coding on the statement indicates they are operating out of Florida.

I got the 9.75 charge on my visa today....will dispute it tomorrow..
has anyone had recurring problems with this?   If I have to cancel the card it takes hours to notify all the auto payments I have set up on this....it's a nightmare so If it's not a recurring problem I'd rather not cancel it....

I got a fraud alert from Visa for this merchant: Form Limited, a DVD Rental business. The amount was $9.80.  I had to cancel the card immediately.  The only way this happens is some merchants credit card files have been compromised.  So everyone who has this bogus charge has probably been doing business with the same online retailer.  Who knows who it is.  I remember once when Egghead got hit, it cost millions after all the bad charges were reversed.  Citbank Visa must be on to them because the fraud alert was within hours of the charge.

Nahh, I tried to ignore the first faudulent charge with another vendor, but this new charge showed up 3-4wks later.

For recurring payments, some card supplers (like FIA) let you set up separate card #s online for each recurring charge, so if your main card gets hit, your recurring charges don't have to change.  Cool.

This is an update from my last entry.  I called VISA and they said I should contact the bank who issued my card and to file a complaint with the FTC (Federal Trade Commission).  I contacted my bank (actually credit union)this morning and they immediately canceled my card and will issue a new one.  They assured me that I would get my $9.85 back also.  I told them about this website and she actually printed out all of these complaints and forwarded them to the credit department for their information!  She said that these crooks will often try several small charges first to make sure a card number is "good".  When a transaction or two goes through, they will then charge thousands of dollars to your number and then immediately "dump" it.  I would suggest that you all have your cards canceled now and get a new one issued.  I would also suggest that you all file a complaint with the FTC.  These are the people who can actually prosecute crooks like this.  If they don't get any complaints, no one will ever be caught and prosecuted!  Take the time to file a complaint!
I use my credit card a lot for both on-line and off-line purchases. Any dishonest worker who takes your card information can sell it to other crooks which is how this type of situation comes about.  I still will never use my credit card in a restaurant or similar place where the waiter or waitress walks away with your card for 5 minutes and then brings it back to you!  They can get all the info off your card and then sell it! When you buy things on line, you pretty much have to hope that the company you are buying from has screened their workers well, but I sure wouldn't count on it!  Most of these people are earning low pay and the company they work for only really cares if your number is good and they get their money.  In other words, it's up to you to pay close attention to your charges and stop anything that's unusual before it gets out of hand.   Good luck to you all.

I keep track of all my expenditures using Microsoft Money and have done so for years.  When I came across this charge I knew it was a fraud.  I am the only user of my credit card.  These people aren't as stupid as most crooks.  (They are still stupid however.)  They know that a lot of people won't even notice a charge that is less than $10.  If they do this to a few hundred thousand people though, it adds up quickly.
 What makes them stupid is that if they only put in half the effort that they do to rip people off, into a legitimate business venture, they would be far more successful!  What a bunch of morons!

The number belongs to West Asset Management. They always leave an automated recording when they call and it gets stinking. I found their customer service number: 1. A few moments ago, I too had a $9.80 charge show up. Bastards. This is infuriating and I sure wish I could figure out exactly why and how this happened. I did not authorize any charges this morning and know nothing about this company. Going to the bank today to fill out a dispute form. The transaction says FORM LIMITED CORAL GABLES FLUS 025441 on my bank deal. A-holes. I am gonna try to get them and sue their pants off.

I just got my credit card bill yesterday with an unauthorized $9.75 charge.  I called the credit card company right away to dispute. I was told it was some kind of a video charge but they are sending me a dispute form also.  In addition I tried calling the 888 number and only got an automated voice mail.  Thanks for all of your comments-it makes me feel a bit better knowing I'm not the only one out there.

I got a bogus charge for $9.75 on 4/29 on an otherwise inactive citibank account. I called citibank to ask they gave me this number to double check if I made the charge. When I called, I first got a extended phone keypad tone. The second time I called, I got a recorded answering machine, that told me to leave a message. Then I saw this site and called citibank to dispute the charge.

I also had a charge for $9.75 on this months bill.  I called Visa they said it was point of sale in Florida-I am in NY. I did use the card for an online purchase the same day-I think it is a trojan.
