Received a FAX to our company also.
Our approach is to send one 'nice' e-mail. (In this case, the spammer's contact information is shown on www.sportality.com/2007/gx2008.cfm?id=2496.)
We have recently received a fax from you attempting to promote your company, products and/or services; unfortunately, the only result is that these faxes just annoy many potential clients, tie up many companies? employees, and their networks trying to get rid of your spam faxes.
The belief that recipients (and thus potential clients) will call or access an opt-out, toll-free telephone number or website is comparable to replying to e-mail spam ... it just tells individuals that our fax number is legitimate and working.
I encourage you to use only positive approaches to promote your company, and stop sending faxes to _______.
By the way, your fax made it to our recycle bin ... not to any decision-makers.

I can usually find an e-mail for the spammer by doing an Internet search on their toll-free number. I also cc the advertising company contact if the spammer is selling their spamming as a service.
Since I keep all of these e-mails in a separate directory, I can do a search if something comes in that looks familiar; if I have already sent an e-mail requesting removal and receive a second (or third ...), I will forward the original note to the spammer's contact e-mail and cc every other e-mail address I can find for the spammer.
Occasionally I will hit a nerve and get a reply about the 'service' they are providing ... but it (as with the spam) does not make it past my desk.
