We received a fax on 19 Feb 2009 requesting we sign an "Authorization to release financial information" form and fax it to this number.  It looks like they have corrected most of the typos and has the apearance of DOT letterhead.  Fax subject line is:  "From Govt Purchasing/US-DOT".  Our company name was just different enough to raise red flags.

FAX received from "Govt Purchasing/US-DOT" on 2-18-09 @ 1:19pm, 2 pages. "Dear Sirs,

Our records show that you are currently registered as a prospective contractor for procurements issued by the U.S. Department of Transportation. However after reviewing your record we have noticed that you have not submitted your financial information release form.
Your financial institution's privacy policy may not allow it to release your financial information even to government institutions without your consent; therefore we must have such form on file before we can move on with any procurement decisions.
In March 2009, the procurement plan for the third quarter of 2009 will be finalized. To be eligible for procurement with the U.S. Department of Transportation you must submit the abovementioned form to us by fax at 202-747-0908.
Please make sure your fax is preceded by a cover page so we may correctly identify your company.

I just answered my own question.  When I entered 202-330-5749, I found more information regarding this.  You are right.  It is a scam.

got a FAX requesting information be called in to this number. Address was U.S. Dept of Transportation and www.dot.gov

It's a scam! Do not release any info.
