The "North American Food Safety Institute" http://www.premierfoodsafety.com/ is a for-profit training center. They want to either use the "survey" to show that Americans are ao alarmed about food safety that their target market needs to get training; or else they just want to sell you training.

Probably Schwann's or a similar company doing a "safety survey" but actually collecting your info to try and sell you stuff.

It was a pre-recorded "45 second" survey from the North American Food Safety Institute. At the end, it says that they'll be in the area giving away free samples and asked if I'm interested in having them come here.
