They are Premier Health Care, American Trade Association(ATA) or Smart Data Solutions(SDS). I do not know their relationship. But it is a phone fraud. Do not give them any information. I was cheated by about 600 dollars.

The only way to beet them is to fax them about 20 times from the fax number you received these on. I bet they remove you then...I just did

I get 'affordable health insurance' faxes that say to call that same number. It says to call 866 317 4701 to stop receiving the faxes, so maybe it would work for your phone
