By Google'ing "645 HH" I found this, since I didn't find anything looking up the phone #.  FYI, I'm from Illinois and received an "Official Document" from them.  (Insert chuckle here.)  There is NO return address; the closest is the presorted postmark which says Oklahoma City, OK, permit No. 2018.  In reading some of the comments above (too many to read all), the value of the "voucher" (for 2 round trip airline tickets) that "must be certified to be valid" is now $1,450.00.  It also says "This is not a timeshare or land sales offer.", & that if I don't claim my "award, it will be transferred to the alternate".  Well, good luck to the alternate since I won't be responding...  Into the garbage it goes.  It seems there might be yet another phone # they're using:  844-828-2427

I just received a voice mail from this guy named vito platt who couldn't speak english at all he said he was calling from the state investigation department and that I had a lawsuit against me and my SS# he also said I needed to call back and give him my address and employer name to get my subpoena papers for the lawsuit.

Claimed to be from FDIC name is Armando Alejandro. Calling in regards to a debt from Advanced Cash America. Told them I was reporting them to my local law enforcement and referred them to my lawyer.

received 5 calls in 10 minutes.  The first 3 calls were from a Kansas area code.  I knew it was a scam since I don't owe ANYBODY including having an outstanding loan.  I told the man, who spoke terrible English, just that.  Couldn't shut him up so I hung up.
One minute later he called again.  I repeated what I had told him the first time and told him to remove my # from his calling list.  Then hung up..  Less that an minute later he called again.  I did not pick up.  He left a message to call him back. A few minutes later, a call came in from an CA area code  It was the same guy.  I hung up.  He called again from the CA #.  I ignored it.

Received my award notice from Oklahoma City the scam lives on this time the phone # ls 1-866-949-7219  WATCH OUT

I just got mine today from the same 645--HH. This time with the number 1-866-931-8017.

I live in the Milwaukee, WI area.  My "official document" appears to be sent from Oklahoma City as well, with no other identifying information about the company, just "Sincerely, Awards Department, 1-866-537-4552."  I am supposed to call to claim my 2 airline tickets worth "up to $1,298."  I have done several searches trying to locate this number, with NO results.  The most interesting thing I found was one site that had it listed as both a cell number and a land line at the same time... that screams scam to me!!!

5/25/12 in Vancouver WA  From 645-HH Award Notification 2 Roundtrip Airline Tickets Call 1-866-217-2969 did not call and have not received any calls just this in the mail. My 83 year old mother might have fallen for this so I'm glad she showed it to me.

Got one today in Lake Worth Florida. Call back number is now 1-866-223-3197, and the "airline" claims to be "US Airlines". It was enough to make my brain think "US Airways".  The number does not show up anywhere on the net except for these pages. Thanks for the extensive page folks. :)

People who do these things should burn in hell or at least fined at the very least. we should propose a bill to fine telemarketers aand scammers.

Got ours today in CA.   Still using Form 645--HH, but now switched its address to a PO 20389 in West Palm Beach,FL, its airline to AirTran, AirTran and its call back number to 855-402-0812.   My official issued award notification is No. 0412.   I wonder if they at least change this number with every notification.

I figured from the official look of the mail - yet the disclaimer that it wasn't government mail - that this was junk, but was curious what the fine print was.  So, I "googled" the 866 number, and found nothing, but then tried the "Form" number, and found this.  The new number they're using is 866 218 7143.  This is IN, too, so it's moving on.

Just got one today from jetBlue.  645--HH, call back number 1-877-209-4922. from San Diego.  Awards Department must be nice to give out so many of these. (yeah right!) NOT ENDORSED BY JETBLUE OR ANY OF ITS AFFILIATES!! I hope you burn it after reading!!

I recieved one of these letters today, in San Diego, CA.  Same as Fred's last one.  Except from JetBlue airlines.  Same form, claim number and phone number.

Latest Award Notification from Delta Airlines and form 645--HH, Claim Number 3914 for 2 Round Trip airline tickets - only now they're using a different phone number: 1-877-209-4922.  Oh, and they are making the Announcement Type:  FINAL NOTICE.  REALLY?  Do you PROMISE???

I bet if I called Delta Airlines, they'd tell me they don't HAVE an Awards Department Division or Awards Department.

Got a fax from1-877-854-0265 about a vacation package for American Airlines Employees . I called
this number back and the fellow asked me if I wanted to book a vacation I replied no - I am not an
employee of American Airlines. Then he asked me if I had money and I replied no - so he hung up.
There is a fax removal number that is busy when called - it is 1-888-393-1379.
