Advertisement for Health Care Benefits.  Ad says to call 888-808-0389.  They do not answer with a company name but when asked they will say it is "Superior Marketing".  The ad also says you can call 1-866-614-1061 to be removed from their distribution. This number is answered by an automated response from the "Fax Removal Hotline".
