Mohave - Bullhead City, AZ).  It's tied to some of the offices in an ancillary building not on the main switchboard, so the call probably came from one of the departments there - physical therapy, speech therapy, community relations, etc.  The call could have been regarding an appointment.  Some listings still show it as Colorado River Medical Complex, the building's previous name.
