Today received fax saying fill out and sign a document in order to work for us dept of transportation, then fax to above number. Doc requests bank account #, ein #, authorized signature. if you do it you will see your signature again on a big bucks wire transfer authorization sent to your bank. fax transmittal header says Equifax, but letterhead is U.S. Dept of Transportation and lists D.C. address and www.dot.gov. isn't there someone that investigates this c.r.a.p.?

We had the form sitting on the fax when I came in this morning. Equifax at the top as most are posting for the US DOT for Julie Weynel - Senior Procurement Officer.
Glad I typed in the fax number and found this.
Keep reporting these things so other will not fall prey to the criminals.

Got a fax with a request for our bank acct info as well, this time with a U.S. DOT logo and requester id. The fax header shows the "Equifax - ADM R Dept" mentioned before, though. The requested return fax line was 202-480-2590. A quick search turned up this site, of course.
I sure hope these scammers get electrocuted by their own fax machine...

We received the same thing. It is somewhat realistic because it does contain accurate company information. Our rep that it was addressed to called her government contacts and they said it was a total SCAM and not to respond.

Received the Equifax - Government Division fax today. The fax mast header read "Equifax ADM R Dept." Letter signed by Nick Stanley. Letter requested our company's authorization to release credit financial data so we can be registered as a prospective US Federal Govt contractor.

I work for an information security department at a regional bank. We had a customer receive a fax from this number purporting to be from Equifax, one of the credit reporting agencies. It asked for the business customer's account number and other information. The customer faxed it back to this number. The next day, a fraudulent wire transfer request was sent to the bank attempting to wire $26,000 out of the customer's account. We verified the request to be fraudulent and did not wire the money. The customer's signature was forged on the request and was identical to the signature the customer made when he faxed the form back to the fraudsters.
Do not respond to faxes from this fax number!

Client got same fax datd November 1, 2011 with the above phone number.202-480-2590 as fax wanting banking information. Client does not do any government contracting and it had a wrong company name.

This is a scam fax posing as Equifax. Someone from our company sent the information, and yesterday a company from Russia tried to wire $28,000 from our bank account. They made fax letterhead in our company name and programmed their fax to show as our phone number. They attached a fake invoice and lifted the signature off of the form we sent them. Fortunately our bank must call and verify all outgoing wires. We have since closed the account.

just recieved a fax at my company from this "Jane Lansen", as a construction company and with the economy the way it is, most of what we are working on is government work. i questioned it mainly because it had a date of 10/27/11 and i received it today 11/08/11 and also because there was no phone number on it. googled the number and jane lansen and found this page. you can't trust that anything is what it seems anymore!

The Better Business Bureau has issued an alert regarding this fraud. Also, Equifax has posted a notice on their site informing people NOT to respond to this fax.

received fax as well to fax credit and bank info to 202-480-2590. Must be scam. Equifax doesn't request your bank info. Plus they would provide a number for you to contact. If not, then not legitimate.

Received that fax. Tell tale signs that it's crap: 1) No contact telephone number. 2) Spelling mistakes. 3) (202) is not an Atlanta area code.
When in doubt over the validity of a fax or phone call? Hang up and call the company that requested the info.
Wrote smart a** comment on cover sheet and faxed to the listed fax number.

Received the same faxed letter requesting information be faxed to 202 480-2590. As a General Contractor, it is not unusual to receive requests for information. I found it suspicious there wasn't a telephone number to call with questions.
I spoke directly to Equifax in Altanta, GA and was told it is a bogus letter and has nothing to do with them. The women at Equifax even knew the name "Jane Lansen" appeared on the fax I received.

What a devious way to get you to fax your bank information to a total stranger!
They have mis-spelled our company name and have an address hasn't been used for 10 years.
Also, no phone number to call "Jane Larsen, Senior Credit Analyst" to verify this request.

Just got a fax from them too. It's pretty sneaky to add our DUNS number and SIC codes on these forms as well. We just signed on to receive government jobs. Thanks everyone.

Received a fax scam as stated above. Researched Equifax website for information to verify and found nothing. Then googled the fax number 202-480-2590 found these posts. We are also in the process of bidding a government project. Thanks for the information!!!

Just received this same fax from Equifax. Requesting EIN, Bank Name, Branch Phone, Branch Fax and Operating account number for a credit check of government contractors.
We actually do some government work so they may be phising a government database of some type.
Appears to be a phising scam. I feel like writing something nasty on the form and faxing it back to the scam artists.

We just received a fax as described above. I called Equifax and the fax number 202-480-2590 is not their fax number and they are not sure this letter did not come from them. Nobody seems to know what the other departments are doing. DO NOT COMPLETE THE FORM AND FAX. SCAM ALL THE WAY.

I got this scam fax too, saying reply to 202-480-2590. It looked suspicious to me as a former Attorney General employee who dealt with this type of scam regularly. My business has no need of government contracts. Same scam appeared in March, with fax Numbers:

Received the same fax as Suz. Indicated it was necessary to complete in order to be eligible for procurement with the US Federal Gov't. (George Larsen, Senior Credit Analyst).
