This scam has moved to another phone number. 202-747-6578

I also got the fax.  We were processing them as new customer so I filled out the request.  They require the cover sheet so they can use it. My bank just called me with almost the exact same request/demand.  I had it faxed to me so that I could see what it was.  They have signatures along with the bank info - will have to close that account and open a new one.  I will never send bank info without checking the fax number first.

I too got a fax, only thing is I just updated my file with DOT, so I filled out the request. My bank just called and said that in a fax to the branch, I am demanding to "WIRE" $20,000 to someone in RUSSIA! My bank stopped it! BEWARE!

I received a fax at my business requesting financial information for my company that was supposedly from the U.S. Department of Transportation  even though we have never done nor requested to do business with the agency.

The Fax number it appears to have come from and which it instructed us to use was 202-747-5709.

The official looking fax stated:

Dear Sirs,

Our records show that you are currently registered as a prospective contractor for procurements issued by the U.S. Department of Transportation. However after reviewing your record we have noticed that you have not submitted your financial information release form.

Your financial institution's privacy policy may not allow it to release your financial information even to government institutions without your consent; therefore we must have such form on file before we can move on with any procurement decisions.

In April 2008, the procurement plan for the second semester of 2008 will be finalized. To be eligible for procurement with the U.S. Department of Transportation, you must submit the above mentioned form to us by fax at 202-747-5709.

Please make sure your fax is preceded by a cover page so we may correctly identify your company.
