This number used to belong to a florist, but now its part of a Health Care Insurance SCAM. Premier Health Care is just a marketing company and Association for Lifestyle Reform (ALR) is the principle. You can complain to the FCC. The owner of this company is a Mr. Brad Wessler, who happens to be a licensed insurance agent in California [Lic. No. 0670952] According to the California Department of Insurance his business address is: 6251 Schafer Ave. #O, Chino, CA 91710.

The Government actually made this legal as long as there is an OPT OUT option on the bottom of the FAX - Our government~ Try calling that OPT number - 1 you are giving them your phone number to bother you with calls
2 there are already formal complaints and actions on most of the receipts of faxes you receive so, obviously they realize it is just piece of so call formal complaint paper sent to them with no one actually pursuing the SUE status.

So tottenkof, if you provided "John Smith" with your fax number, all you did is confirm to him that your number is valid fax number opening you up for more faxes in the future. Try this instead next time. Tell them you will give him your name and he can simply use that to remove your number. He won't be able to do that because they don't maintain a list of names, only fax numbers.
Most of these health care offers are nothing more than insurance brokers. They take a fee on for lining you up with a real life insuance company. Just ask the guy next time if his company is licensed to sell insurance in your state. He will then tell you that they are not selling insurance, the five or six companies that they work with are licensed.

Junk Faxes received all hours of the day on my home fax machine. The number is unlisted and I'm on the Do Not Call Registry.
No Fax Origination Number of fax sheet.
Selling healthcare plans. DBA as Excellent Healthcare Plans and Premier Healthcare.
Called to request removal from their list. Talked with a "John Smith" who was a real joker.
Sounds like a scam.
