They aren't asking for your medical history. They are asking for your opinion regarding services that you received while in the physicians office.
IE: Was the staff helpful?
Was the Physician on time and attuned to your concerns?
etc.
... It has NOTHING to do with your medical history.

Someone from this number keeps calling me and they never leave a message. I don't answer calls from people I don't know, and their persistence is getting annoying.

Haven't done any recent business with them.
I have no idea why they called me.
