Be sure you enter bogus info just to piss them off! I believe they are phishers. The first clue was that they don't have an https:// URL when you are entering your personal data.

908-731-1287 is definitely a scam. The 'company' website (http://iphone5tester.mobi) event states
"SampleRewardsOnline.com is an independent rewards program for consumers and is not affiliated with, sponsored by or endorsed by any of the listed products or retailers."
Hence, Apple Inc. is clearly not associated with this organization, nor is Apple looking for iPhone testers through this website.
